HIPAA Notice of Privacy Practices — Pure Life Longevity

Our duties. Pure Life Longevity is required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information.

  • Treatment — to provide and coordinate your care, including with other providers, labs, and pharmacies involved in your care.

  • Payment — to obtain payment for services, such as providing a superbill at your request.

  • Health care operations — for activities necessary to run the practice, such as quality and record-keeping.

  • As required or permitted by law — including public health, legal, and safety purposes permitted under HIPAA.

Most uses and disclosures for marketing, as well as any sale of PHI, require your written authorization. Other uses not described in this notice will be made only with your authorization, which you may revoke.

Your rights. You have the right to: request access to and a copy of your records; request corrections to your records; request restrictions on certain uses and disclosures; request confidential communications; receive an accounting of certain disclosures; receive a paper copy of this notice; and be notified of a breach affecting your PHI. Complaints. If you believe your privacy rights have been violated, you may file a complaint with the practice by calling 978-225-3391.

 or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Changes to this notice. We may change this notice and make the new terms effective for all information we maintain. The current notice will be posted and available on request.
